1. Citizen Crime Reporting
This form needs to be printed and completed prior to dropping off at the police station (hours of 7am to 7pm). This form is only to be used for minor thefts, wilful damage, lost wallet/purses, etc. It is NOT to be used for crimes where violence is alleged, robbery, Break and Enter, etc.

2. Police Information Check: Consent to Search and Disclosure of Personal Information. This form needs to be completed and brought to the Lethbridge Regional Police station, you cannot mail these in. You must provide two pieces of identification one of which must be photo. Identification needs to be photocopied and originals produced with the application. DO NOT SIGN THE FORM, this must be done in front of police staff.

3. Request to access information: Alberta's Freedom of Information and Protection of Privacy Act allow individuals to access information about themselves that organizations may have. Many records do not require this form for access. To determine which information you will need this form for; please contact the F.O.I.P Coordinator at 330-5059.
